Affiliations & Insurance

Boulder Valley Center for Dermatology is proud to be an independent and locally owned practice, not affiliated with a hospital or other healthcare system. If we need to provide visit notes to your primary care provider, be sure to give us that information before you leave.

Insurance

Insurance plans change regularly and, while we participate with many different PPO and HMO plans and accept traditional indemnity insurance, it is your responsibility to verify coverage.   Ask your plan if Boulder Valley Center for Dermatology is:

  • in their network
  • requires a referral to a specialist, like a Dermatologist, from your Primary Care Physician

If you are not sure about our providers’ status with your insurance company, please check with your insurance company or your employer’s benefit representative prior to your appointment.

Financial Policy

We are dedicated to providing the best possible care and service to you and want to insure your complete understanding of our financial policies.

We have contracted with many health insurers and health plans to accept assignment of benefits. If we have a contract with your insurance company, as a courtesy, we will file your claim.  You will be responsible for your deductible, any co-insurance or co-pays.  You will be required to pay for these when you arrive for your appointment.

Some insurance plans require referrals to specialists, like Dermatologists.  If a referral is required, call your Primary Care Physician prior to your appointment to obtain this referral.  If a referral is required and has not been received by our office when you arrive, the visit may be rescheduled, or, if you decide to keep your appointment, you may be responsible for payment.

All health plans are not the same and do not cover the same services.  In the event your health plan determines a service is not covered, you will be responsible for all charges.  Payment is due upon receipt of the statement from our office.

Most cosmetic procedures are not covered by insurance and payment is the responsibility of the patient.
